Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a long-term/rehab environment
• Administer medications and treatments accurately and document them in the medical record
• Monitor residents’ clinical status and communicate changes promptly to the care team
• Support activities of daily living and promote residents’ independence and dignity
• Collaborate with RNs, therapists, and support staff to create safe, effective care plans
• Maintain clear, timely clinical documentation and ensure regulatory compliance
• Participate in shift handoffs and interdisciplinary meetings to ensure continuity of care

Qualifications

• Active Licensed Practical Nurse (LPN) license in Massachusetts (required)
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable working in long-term care or rehabilitation settings; prior LPN experience preferred
• Strong communication skills and a team-oriented approach
• Ability to pass required background checks and health screenings

Education

• Completion of an accredited practical nursing program (LPN diploma or certificate)
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and pressure injury prevention
• Familiarity with electronic medical records and charting
• Skilled in clinical assessment and vital signs monitoring
• Good prioritization, time management, and teamwork skills
• Comfortable with resident-centered, compassionate care
