Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents in a long-term/rehab environment
- Administer medications and treatments accurately and document them in the medical record
- Monitor residents’ clinical status and communicate changes promptly to the care team
- Support activities of daily living and promote residents’ independence and dignity
- Collaborate with RNs, therapists, and support staff to create safe, effective care plans
- Maintain clear, timely clinical documentation and ensure regulatory compliance
- Participate in shift handoffs and interdisciplinary meetings to ensure continuity of care
Qualifications
- Active Licensed Practical Nurse (LPN) license in Massachusetts (required)
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Comfortable working in long-term care or rehabilitation settings; prior LPN experience preferred
- Strong communication skills and a team-oriented approach
- Ability to pass required background checks and health screenings
Education
- Completion of an accredited practical nursing program (LPN diploma or certificate)
- High school diploma or GED
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and pressure injury prevention
- Familiarity with electronic medical records and charting
- Skilled in clinical assessment and vital signs monitoring
- Good prioritization, time management, and teamwork skills
- Comfortable with resident-centered, compassionate care
