Roles & Responsibilities

• Perform thorough evaluations and develop individualized treatment plans for adults, geriatrics, and adolescents
• Provide evidence-based interventions for orthopedic and neurologic conditions including vestibular care
• Conduct Fall Risk Assessments and document findings clearly in NextGen Case Management
• Deliver post-operative care, manual therapy, and functional training to help patients regain independence
• Meet expected productivity targets (posting indicates ~55 visits per week) while maintaining excellent patient care
• Collaborate with the interdisciplinary team, communicate progress to patients and families, and participate in care coordination
• Maintain accurate, timely documentation and adhere to clinic policies and quality standards

Qualifications

• 1+ years of clinical experience (explicitly required)
• Active Physical Therapist license in Delaware or ability to obtain state licensure/compact privilege
• Experience with NextGen Case Management (listed as a primary skill)
• Proficiency in Fall Risk Assessment and experience treating vestibular, Parkinson's disease, CVA, and concussion cases (preferred/optional per posting)
• BLS/CPR certification (or willingness to obtain)
• Strong clinical reasoning, patient education skills, and documentation accuracy
• Comfortable meeting a higher-volume outpatient caseload and following a business-casual dress code

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent clinical degree

Knowledge & Skills

• NextGen Case Management documentation
• Fall risk assessment and mitigation strategies
• Vestibular rehabilitation and vestibular testing/management
• Orthopedic and post-operative rehabilitation (rotator cuff, total joint replacement, etc.)
• Neurologic conditions management (Parkinson's disease, CVA, concussion)
• Manual therapy techniques, therapeutic exercise prescription, and patient education
• Time management and caseload/productivity planning
• Effective interdisciplinary communication and care coordination
