Roles & Responsibilities
- Provide compassionate, hands-on care to residents — assist with ADLs and promote dignity
- Administer medications and treatments accurately and safely according to orders
- Monitor and document vital signs, patient status, and changes in condition in the chart
- Collaborate with the interdisciplinary team to support care plans and rehab goals
- Support wound care, skin integrity checks, and timely reporting of concerns
- Educate residents and families on care routines and follow-up instructions
- Maintain a safe, clean environment and follow infection control protocols
Qualifications
- Current LPN license in Massachusetts (required)
- Current BLS/CPR certification (required)
- Completion of an accredited LPN program
- Current immunizations and TB screening as required by facility policy
- Strong communication, empathy, and teamwork skills
- Previous experience in long-term care or rehab settings preferred
Education
- Completion of an accredited Practical Nursing (LPN) program
Knowledge & Skills
- Medication administration and accurate documentation
- Basic wound care and skin integrity knowledge
- Comfort assisting with ADLs and mobility support
- Familiarity with electronic health records/charting
- Infection control practices and safety awareness
- Good prioritization, time management, and bedside manner
