Roles & Responsibilities
- Provide hands-on patient care with compassion and clinical confidence
- Administer medications and treatments safely according to physician orders
- Monitor and document patient status and communicate changes to the care team
- Assist with ADLs (activities of daily living) and support patient mobility
- Collaborate with RNs, therapists, and facility staff to create a seamless care plan
- Contribute to a positive, resident-centered environment that prioritizes dignity and respect
Qualifications
- Active LPN (Licensed Practical Nurse) license in Massachusetts (required)
- Current Basic Life Support (BLS) certification or willingness to obtain
- Experience in long-term care, rehab, or skilled nursing settings is a strong plus
- Excellent communication skills and ability to work well in a team
- Reliable, professional, and patient-focused with strong documentation habits
Education
- Completion of an accredited Practical Nursing (LPN) program or equivalent clinical diploma/certificate
Knowledge & Skills
- Skilled in medication administration and basic wound care
- Comfortable with electronic charting and clear documentation
- Knowledge of infection control practices and resident safety protocols
- Strong observational skills to recognize changes in condition
- Team player mentality with good time management and prioritization
