Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a rehab and long-term care setting
- Administer medications and treatments accurately and document per facility policy
- Monitor resident condition and communicate changes promptly to the care team
- Support rehabilitation plans by assisting with mobility, ADLs, and restorative care
- Collaborate with nurses, therapists, and physicians to deliver coordinated care
- Educate residents and families about care plans, medications, and safety
- Maintain accurate electronic and paper records in a timely manner
- Adhere to infection control practices and facility safety protocols
Qualifications
- Current LPN license in good standing (required)
- Completion of an accredited LPN program or equivalent (required)
- BLS/CPR certification (required or obtain upon hire)
- Experience in long-term care, rehabilitation, or skilled nursing preferred
- Current immunizations and eligibility to pass background and health screenings
- Strong communication skills and a collaborative mindset
Education
- Graduate of an accredited Licensed Practical Nurse (LPN) program
- High school diploma or equivalent
Knowledge & Skills
- Medication administration and accurate MAR/charting skills
- Basic wound care and dressing change familiarity
- Comfortable with ADL assistance and restorative nursing techniques
- Ability to observe, assess, and report clinical changes
- Familiarity with electronic medical records and documentation standards
- Good time management, prioritization, and teamwork abilities
