Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Evaluate adult, geriatric and adolescent patients to identify impairments, functional limitations, and goals
- Develop and implement individualized treatment plans emphasizing post-operative care and musculoskeletal recovery
- Perform fall risk assessments and apply prevention strategies to keep patients safe at home and in clinic
- Manage a high-volume caseload (~55 visits/week) while maintaining thorough, timely documentation in NextGen Case Management
- Provide education and self-management strategies to patients and caregivers to maximize recovery and prevent recurrence
- Collaborate with physicians, case managers and interdisciplinary team members to coordinate care and optimize outcomes
- Monitor patient progress using outcome measures and adjust plans of care to achieve measurable improvement
Qualifications
- Active Physical Therapist license in Delaware or eligibility to obtain state licensure quickly
- Minimum 1 year of clinical physical therapy experience (outpatient experience strongly preferred)
- Comfortable treating a broad range of musculoskeletal and post-operative conditions (e.g., rotator cuff, total joint replacement, meniscus, spine)
- Experience with NextGen Case Management or similar electronic health record documentation systems
- Basic Life Support (BLS) / CPR certification (or willing to obtain upon hire)
- Ability to manage an active caseload (~55 visits/week) with strong time management and documentation skills
- Professional appearance consistent with dress code: khakis or dress pants, collared/business casual shirt, closed-toe shoes
Education
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy preferred; entry-level professional degree required by state licensure
Knowledge & Skills
- Strong knowledge of post-operative protocols and rehabilitation progressions
- Skilled in fall risk assessment and preventive interventions
- Proficient with musculoskeletal evaluation and manual therapy techniques
- Comfortable with vestibular rehab, Parkinson’s care, CVA and concussion management (optional/bonus)
- Experience using NextGen or comparable case management/EHR systems
- Excellent patient education, communication and interdisciplinary collaboration skills
- Ability to prioritize and manage high patient volume while maintaining quality care
