Roles & Responsibilities

• Conduct clinical screening, assessment, diagnosis, treatment planning, brief therapy, and continuing care for clients from birth through elderly
• Lead and coordinate inpatient discharge planning, including placement coordination for ALF and transitional care
• Facilitate or co-facilitate patient care conferences with multidisciplinary teams
• Provide clinical support and referral coordination for chemical dependency and mental health needs
• Advocate for clients and assist with complex matters such as Power of Attorney when appropriate
• Maintain accurate, timely, and compliant documentation to meet state, federal, accreditation, and hospital billing standards
• Collaborate with community partners and corporate resources to ensure optimal continuity of care
• Participate in and support staff trainings, quality management activities, and accreditation compliance efforts

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or related discipline
• Licensed or in process of becoming Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT)
• Valid State of Alaska driver license or ability to obtain within 6 months of hire
• Must be 21 years of age
• Clinical competency required within 3 months of hire and maintained every three years
• Completion of Clinical Competency Assessment - SEARHC

Education

• Master's Degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution

Knowledge & Skills

• Discharge planning and care coordination
• Interviewing techniques and crisis intervention
• Clinical assessment, diagnosis, and treatment planning
• Case management and documentation standards for billing and accreditation
• Patient advocacy and skill-building strategies
• Experience with ALF/transitional placement coordination and provider enrollment
• Familiarity with tribal health, cultural wellness, and multidisciplinary collaboration
• Ability to prepare clear, concise reports and professional chart notes
