Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings using sound clinical reasoning
• Develop and implement individualized treatment plans to help residents reach their highest functional potential
• Collaborate with the interdisciplinary team to ensure coordinated, person-centered care
• Document interventions and progress accurately and in accordance with facility and regulatory standards
• Participate in care conferences and contribute to ongoing quality improvement efforts
• Advocate for residents’ mobility, safety, and independence while coaching families and staff

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical skills with demonstrated experience in patient evaluation and treatment planning
• Excellent communication skills for working with residents, families, and interdisciplinary teams
• Skilled nursing or post-acute rehab experience preferred
• Dependable, flexible, and team-oriented with a commitment to compassionate care
