Roles & Responsibilities

Provide direct nursing care to clinic patients within the scope of LPN licensure, delivering compassionate, practical care every day
Take and document vital signs accurately and perform initial triage to prioritize patient needs
Handle patient triage calls, manage prescription refills, and complete necessary prior authorizations to keep care moving
Act as the primary liaison between patients, staff, and providers — communicate clearly and keep everyone on the same page
Collaborate with the interdisciplinary care team to coordinate patient care and ensure seamless handoffs
Give direction to CNAs and schedulers and receive direction from providers, coordinators, clinical managers/supervisors and RNs
Maintain required documentation and follow current state and federal licensure continuing education standards

Qualifications

Current State LPN license (required)
BLS (Basic Life Support) certification (required)
Graduate of an accredited Nursing Program (LPN)
Comfort with taking vital signs, triage, prescription refills, and completing prior authorizations
Strong communication skills and ability to work collaboratively with clinical teams and patients

Education

Graduate of a Nursing Program (LPN) — completion of an accredited practical nursing program

Knowledge & Skills

Patient care and clinical assessment (vital signs, basic triage)
Care coordination and interdisciplinary collaboration
Communication and liaison skills with patients and providers
Delegation and supervision of CNAs and support staff
Experience with prescription refills and prior authorization workflows
Adherence to licensure and continuing education requirements
