Roles & Responsibilities

• Provide skilled occupational therapy services to residents in a skilled nursing rehabilitation setting
• Assess patients' functional abilities and create individualized treatment plans that promote independence
• Deliver hands-on interventions and adaptive strategies to improve daily living skills
• Collaborate closely with nursing, PT, SLP and restorative teams to coordinate patient-centered care
• Document progress clearly and timely to support care continuity and reimbursement needs
• Educate residents and families on strategies to maximize safety and functional outcomes
• Participate in discharge planning and transitions to ensure continuity of care

Qualifications

• Licensed Occupational Therapist (OT) in the state of Michigan or ability to obtain licensure promptly
• Current CPR certification (or willingness to obtain upon hire)
• Experience working in a Skilled Nursing Facility or rehabilitation setting preferred
• Strong documentation skills and familiarity with therapy billing/quality expectations
• Excellent communication and teamwork skills — you’ll be part of an interdisciplinary crew

Education

• Degree in Occupational Therapy (OT) — entry-level qualifications required; advanced degrees welcome

Knowledge & Skills

• Knowledge of functional assessment and ADL training
• Skilled in therapeutic interventions for older adults and post-acute rehabilitation
• Ability to build rapport with residents and families
• Familiarity with regulatory and documentation standards in SNF settings
• Comfort collaborating within an interdisciplinary team and participating in care planning
