Roles & Responsibilities

• Provide compassionate direct nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments safely and document per facility policy
• Monitor resident condition, recognize changes, and report to the interdisciplinary team
• Collaborate with nurses, therapists, and families to support individualized care plans
• Maintain accurate clinical documentation and medication records
• Support infection control practices and a safe, respectful environment for residents
• Assist with activities of daily living and promote resident comfort and dignity

Qualifications

• Current Licensed Practical Nurse (LPN) credential — required
• Active license in Massachusetts or eligibility to practice in MA preferred
• Comfortable working 12-hour day shifts (8:00 AM - 8:00 PM)
• Strong clinical judgment, reliable documentation habits, and teamwork mindset
• BLS/CPR certification preferred or willing to obtain upon hire

Education

• Graduate of an accredited Practical Nursing (LPN) program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and basic nursing procedures
• Patient assessment and monitoring of vital signs
• Clear communication with residents, families, and clinical team members
• Time management and prioritization in a fast-paced skilled nursing setting
• Familiarity with long-term care workflows and infection control best practices
• Accurate charting and comfort using electronic or paper clinical records
