Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately and document thoroughly
• Perform routine assessments and communicate changes in condition to the care team
• Support rehabilitation and daily living activities to promote resident independence
• Collaborate with nursing staff, therapists, and physicians to develop and follow care plans
• Maintain meticulous charting and adhere to facility policies and regulatory standards
• Mentor and support CNAs and other staff when needed to ensure consistent, high-quality care

Qualifications

• Active LPN license (Licensed Practical Nurse) — ability to practice in Massachusetts or obtain state licensure
• Current Basic Life Support (BLS) certification or ability to obtain prior to start
• Prior experience in long-term care, rehab, or skilled nursing preferred
• Strong clinical assessment and medication administration skills
• Excellent communication and teamwork mindset; dependable and professional

Education

• Graduate of an accredited LPN/LVN program
• High school diploma or equivalent

Knowledge & Skills

• Skilled in medication administration and documentation
• Familiarity with wound care basics and restorative nursing practices
• Comfortable with electronic health records and timely charting
• Ability to perform focused patient assessments and identify changes in condition
• Good time management, prioritization, and delegation skills
