Roles & Responsibilities

• Evaluate residents to develop individualized, goal-driven therapy plans
• Deliver skilled therapeutic interventions to improve mobility, strength, and daily function
• Collaborate with nursing and interdisciplinary teams to coordinate care and optimize outcomes
• Document progress clearly and timely in the patient chart to support continuity of care
• Educate residents and families on safe mobility, fall prevention, and home exercise programs
• Monitor outcomes and adjust treatment plans to meet changing needs and regulatory standards

Qualifications

• Active, unrestricted Physical Therapist license (Montana) or ability to obtain state licensure promptly
• Current Basic Life Support (BLS) certification (or willingness to obtain before start)
• Experience in skilled nursing, rehabilitation, or post-acute care preferred
• Strong clinical assessment and manual therapy skills with sound clinical reasoning
• Excellent communication skills and a collaborative attitude toward interdisciplinary care
• Ability to pass facility onboarding and background checks for a rapid start

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy or equivalent considered

Knowledge & Skills

• Skilled in assessment of mobility, balance, transfers, and functional independence
• Proficient with therapeutic exercise, gait training, and assistive device recommendations
• Knowledgeable about documentation standards for skilled nursing and Medicare/Medicaid requirements
• Comfortable with patient and family education and discharge planning
• Able to prioritize caseloads and manage time efficiently in a busy rehab environment
