Roles & Responsibilities
- Evaluate residents to develop individualized, goal-driven therapy plans
- Deliver skilled therapeutic interventions to improve mobility, strength, and daily function
- Collaborate with nursing and interdisciplinary teams to coordinate care and optimize outcomes
- Document progress clearly and timely in the patient chart to support continuity of care
- Educate residents and families on safe mobility, fall prevention, and home exercise programs
- Monitor outcomes and adjust treatment plans to meet changing needs and regulatory standards
Qualifications
- Active, unrestricted Physical Therapist license (Montana) or ability to obtain state licensure promptly
- Current Basic Life Support (BLS) certification (or willingness to obtain before start)
- Experience in skilled nursing, rehabilitation, or post-acute care preferred
- Strong clinical assessment and manual therapy skills with sound clinical reasoning
- Excellent communication skills and a collaborative attitude toward interdisciplinary care
- Ability to pass facility onboarding and background checks for a rapid start
Education
- Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy or equivalent considered
Knowledge & Skills
- Skilled in assessment of mobility, balance, transfers, and functional independence
- Proficient with therapeutic exercise, gait training, and assistive device recommendations
- Knowledgeable about documentation standards for skilled nursing and Medicare/Medicaid requirements
- Comfortable with patient and family education and discharge planning
- Able to prioritize caseloads and manage time efficiently in a busy rehab environment
