Roles & Responsibilities

• Provide compassionate, hands-on nursing care for residents at the skilled nursing facility
• Administer medications and treatments safely and accurately
• Assess resident conditions and communicate changes to the interdisciplinary team
• Document care thoroughly and maintain accurate medical records
• Support rehabilitation plans and assist with activities of daily living
• Partner with families and caregivers to ensure resident-centered care
• Contribute to a positive, professional unit culture focused on quality outcomes

Qualifications

• Current Licensed Practical Nurse (LPN) license (Massachusetts license or ability to obtain state licensure)
• Current Basic Life Support (BLS) certification (or willingness to obtain upon hire)
• Experience in long-term care, rehab, or skilled nursing preferred
• Strong clinical judgment, reliable attendance, and excellent communication skills
• Ability to pass standard facility background checks and health screenings

Education

• Graduate of an accredited Practical/Vocational Nursing program

Knowledge & Skills

• Skilled in medication administration and documentation best practices
• Knowledge of wound care, catheter care, and common rehab nursing interventions
• Proficient in patient assessment and care planning
• Comfortable using electronic health record systems
• Team-oriented with strong interpersonal and problem-solving skills
