Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments safely and accurately
- Monitor patient conditions and communicate changes to the care team
- Document patient care and observations in a timely, accurate manner
- Support rehabilitation goals and assist with ADLs to promote independence
- Collaborate with nurses, therapists, and support staff to deliver coordinated care
Qualifications
- Current LPN license (Licensed Practical Nurse) — active and in good standing
- Comfortable working 12-hour day shifts (8:00 AM - 8:00 PM)
- Strong communication and teamwork skills in a long-term care or rehab setting
- Basic clinical skills including medication administration and documentation
Education
- Graduate of an accredited Licensed Practical Nurse (LPN) program
- High school diploma or GED required
Knowledge & Skills
- Knowledge of basic nursing procedures and vital sign monitoring
- Medication administration and safe charting practices
- Ability to assess patient needs and escalate concerns appropriately
- Strong interpersonal skills for patient and family interactions
- Familiarity with long-term care and rehabilitation workflows
