Roles & Responsibilities

• Provide high-quality, compassionate nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments safely and accurately according to facility protocols
• Assess residents regularly and communicate changes in condition to the care team
• Document care, observations, and interventions clearly in the medical record
• Collaborate with nurses, therapists, and other staff to create individualized care plans
• Assist with activities of daily living while promoting resident dignity and independence
• Support admissions, discharges, and transfers with efficient clinical handoffs

Qualifications

• Active LPN license in Massachusetts (or compact/licensure acceptable per facility policy)
• Current BLS (Basic Life Support) certification
• Previous experience in long-term care, skilled nursing, or rehab preferred
• Strong clinical assessment and medication administration skills
• Excellent communication, teamwork, and resident-centered care mindset
• Ability to pass facility background checks and required health screenings

Education

• Completion of an accredited Practical Nursing program and active LPN licensure

Knowledge & Skills

• Proficient in medication administration and nursing documentation
• Skilled in wound care basics, catheter care, and safe patient handling
• Able to perform timely resident assessments and escalate concerns appropriately
• Comfortable collaborating with interdisciplinary teams and following care plans
• Good time management and the ability to prioritize in a busy rehab setting
