Roles & Responsibilities
- Provide high-quality, compassionate nursing care to residents in a skilled nursing and rehab setting
- Administer medications and treatments safely and accurately according to facility protocols
- Assess residents regularly and communicate changes in condition to the care team
- Document care, observations, and interventions clearly in the medical record
- Collaborate with nurses, therapists, and other staff to create individualized care plans
- Assist with activities of daily living while promoting resident dignity and independence
- Support admissions, discharges, and transfers with efficient clinical handoffs
Qualifications
- Active LPN license in Massachusetts (or compact/licensure acceptable per facility policy)
- Current BLS (Basic Life Support) certification
- Previous experience in long-term care, skilled nursing, or rehab preferred
- Strong clinical assessment and medication administration skills
- Excellent communication, teamwork, and resident-centered care mindset
- Ability to pass facility background checks and required health screenings
Education
- Completion of an accredited Practical Nursing program and active LPN licensure
Knowledge & Skills
- Proficient in medication administration and nursing documentation
- Skilled in wound care basics, catheter care, and safe patient handling
- Able to perform timely resident assessments and escalate concerns appropriately
- Comfortable collaborating with interdisciplinary teams and following care plans
- Good time management and the ability to prioritize in a busy rehab setting
