Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a rehab and nursing setting
- Administer medications and treatments accurately and safely
- Collaborate with the interdisciplinary team to create and follow care plans
- Monitor and document patient conditions, changes, and responses to interventions
- Support patient mobility and rehabilitation activities to promote independence
- Communicate clearly with residents and families to ensure understanding and comfort
- Participate in admissions, discharges, and transfers as needed
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing
- Clinical experience in long-term care or rehabilitation preferred
- Basic Life Support (BLS/CPR) certification required or willing to obtain
- Excellent communication and teamwork skills
- Ability to perform duties with compassion, professionalism, and attention to detail
Education
- Completion of an accredited LPN/Vocational Nursing program; high school diploma or equivalent required
Knowledge & Skills
- Strong medication administration and documentation skills
- Knowledge of infection control and safety practices
- Comfortable with electronic health records and charting
- Ability to prioritize tasks in a fast-paced clinical environment
- Skilled at building rapport with patients and families
