Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a rehab and nursing setting
• Administer medications and treatments accurately and safely
• Collaborate with the interdisciplinary team to create and follow care plans
• Monitor and document patient conditions, changes, and responses to interventions
• Support patient mobility and rehabilitation activities to promote independence
• Communicate clearly with residents and families to ensure understanding and comfort
• Participate in admissions, discharges, and transfers as needed

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing
• Clinical experience in long-term care or rehabilitation preferred
• Basic Life Support (BLS/CPR) certification required or willing to obtain
• Excellent communication and teamwork skills
• Ability to perform duties with compassion, professionalism, and attention to detail

Education

• Completion of an accredited LPN/Vocational Nursing program; high school diploma or equivalent required

Knowledge & Skills

• Strong medication administration and documentation skills
• Knowledge of infection control and safety practices
• Comfortable with electronic health records and charting
• Ability to prioritize tasks in a fast-paced clinical environment
• Skilled at building rapport with patients and families
