Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing/rehab setting
- Administer medications and treatments safely and accurately according to orders
- Perform timely patient assessments and monitor changes in condition
- Document nursing care clearly and thoroughly in the electronic medical record
- Collaborate with RNs, therapists and the interdisciplinary team to create and follow care plans
- Assist residents with activities of daily living while promoting independence and dignity
- Communicate with residents and families with empathy and professionalism
- Maintain infection control practices and ensure a safe environment for patients and staff
Qualifications
- Current Massachusetts LPN license in good standing (required)
- Basic Life Support (BLS) / CPR certification (required)
- Graduation from an accredited Practical Nursing program (LPN/LVN)
- Ability to pass background check and drug screening as required by facility
- Previous experience in long-term care or rehabilitation is preferred but not required
- Strong communication, time management and teamwork skills
Education
- Graduation from an accredited Practical Nursing (LPN/LVN) program
Knowledge & Skills
- Medication administration and accurate charting
- Clinical assessment skills and vital signs monitoring
- Wound care basics and safe patient handling
- Familiarity with electronic medical records and documentation standards
- Infection control procedures and safety best practices
- Patient- and family-centered communication and education
- Ability to work effectively on a busy rehab/long-term care unit
