Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing/rehab setting
• Administer medications and treatments safely and accurately according to orders
• Perform timely patient assessments and monitor changes in condition
• Document nursing care clearly and thoroughly in the electronic medical record
• Collaborate with RNs, therapists and the interdisciplinary team to create and follow care plans
• Assist residents with activities of daily living while promoting independence and dignity
• Communicate with residents and families with empathy and professionalism
• Maintain infection control practices and ensure a safe environment for patients and staff

Qualifications

• Current Massachusetts LPN license in good standing (required)
• Basic Life Support (BLS) / CPR certification (required)
• Graduation from an accredited Practical Nursing program (LPN/LVN)
• Ability to pass background check and drug screening as required by facility
• Previous experience in long-term care or rehabilitation is preferred but not required
• Strong communication, time management and teamwork skills

Education

• Graduation from an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

• Medication administration and accurate charting
• Clinical assessment skills and vital signs monitoring
• Wound care basics and safe patient handling
• Familiarity with electronic medical records and documentation standards
• Infection control procedures and safety best practices
• Patient- and family-centered communication and education
• Ability to work effectively on a busy rehab/long-term care unit
