Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and follow physician orders safely and accurately
• Monitor and record vital signs and patient status, escalating concerns promptly
• Assist with activities of daily living while promoting patient dignity and independence
• Support rehabilitation goals by collaborating with therapists and nursing staff
• Maintain accurate, timely documentation in the patient record
• Follow infection control and safety protocols to protect patients and team members
• Participate in shift handoffs and interdisciplinary care planning to ensure continuity

Qualifications

• Current and unrestricted LPN license (Massachusetts) or compact equivalent where applicable
• CPR/BLS certification or willingness to obtain prior to start
• Prior experience in long-term care, skilled nursing, or rehab settings preferred
• Strong communication skills and a team-first attitude
• Ability to pass facility-required background checks and health screenings

Education

• LPN diploma or practical nursing certificate from an accredited program
• High school diploma or GED

Knowledge & Skills

• Solid practical nursing skills including medication administration and wound care
• Familiarity with infection control standards and resident-centered care
• Comfortable with electronic health records and accurate documentation
• Ability to prioritize tasks and manage time during a busy 12-hour shift
• Collaborative approach working with nurses, therapists and support staff
