Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a rehabilitation and nursing setting
- Administer medications and treatments accurately and safely
- Perform wound care, assist with activities of daily living, and monitor vital signs
- Document resident status, interventions, and progress clearly in the medical record
- Collaborate with the interdisciplinary team to support individualized care plans
- Communicate effectively with residents and their families to provide updates and education
- Support rehab goals by assisting therapists and participating in care conferences
Qualifications
- Current Licensed Practical Nurse (LPN) credential — required
- Active or eligible for Massachusetts LPN licensure preferred
- Current Basic Life Support (BLS) certification or willingness to obtain
- Prior experience in long-term care, skilled nursing, or rehab environments is a plus
- Strong communication skills and a patient-centered approach
Education
- Graduate of an accredited practical nursing program (LPN)
Knowledge & Skills
- Fundamental nursing skills: medication administration, wound care, vitals, ADL support
- Comfortable with electronic medical records and clear clinical documentation
- Ability to work collaboratively in a fast-paced rehab and nursing environment
- Good time management, observation, and critical thinking skills
- Empathy, patience, and strong interpersonal communication
