Roles & Responsibilities

• Provide compassionate, resident-centered nursing care consistent with individualized care plans
• Administer medications and treatments safely and accurately, following facility protocols
• Monitor and document vital signs, clinical changes, and response to interventions
• Assist residents with activities of daily living while promoting independence and dignity
• Collaborate with RNs, therapy staff, and interdisciplinary team members to coordinate care
• Communicate clearly and empathetically with residents, families, and the care team
• Maintain accurate, timely documentation in the resident chart and electronic records
• Follow infection control, safety, and regulatory standards to ensure a secure environment

Qualifications

• Active LPN license (Licensed Practical Nurse) — ready to practice with confidence
• CPR/BLS certification (or willingness to obtain upon hire)
• Comfortable working 12-hour day shifts and adapting to facility schedules
• Strong clinical assessment, medication administration, and documentation skills
• Excellent communication skills and a collaborative attitude with interdisciplinary teams
• Previous experience in long-term care or skilled nursing preferred but motivated new grads welcome

Education

• Completion of an accredited Practical Nursing (LPN) program or equivalent

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic wound care, ADL assistance, and monitoring of vital signs
• Familiarity with long-term care workflows and care plan implementation
• Comfortable using electronic medical record systems for charting
• Strong prioritization, time management, and critical thinking skills
• Compassionate resident-centered approach and effective family communication
