Roles & Responsibilities
- Provide compassionate, resident-centered nursing care consistent with individualized care plans
- Administer medications and treatments safely and accurately, following facility protocols
- Monitor and document vital signs, clinical changes, and response to interventions
- Assist residents with activities of daily living while promoting independence and dignity
- Collaborate with RNs, therapy staff, and interdisciplinary team members to coordinate care
- Communicate clearly and empathetically with residents, families, and the care team
- Maintain accurate, timely documentation in the resident chart and electronic records
- Follow infection control, safety, and regulatory standards to ensure a secure environment
Qualifications
- Active LPN license (Licensed Practical Nurse) — ready to practice with confidence
- CPR/BLS certification (or willingness to obtain upon hire)
- Comfortable working 12-hour day shifts and adapting to facility schedules
- Strong clinical assessment, medication administration, and documentation skills
- Excellent communication skills and a collaborative attitude with interdisciplinary teams
- Previous experience in long-term care or skilled nursing preferred but motivated new grads welcome
Education
- Completion of an accredited Practical Nursing (LPN) program or equivalent
Knowledge & Skills
- Medication administration and safe documentation practices
- Basic wound care, ADL assistance, and monitoring of vital signs
- Familiarity with long-term care workflows and care plan implementation
- Comfortable using electronic medical record systems for charting
- Strong prioritization, time management, and critical thinking skills
- Compassionate resident-centered approach and effective family communication
