Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term/rehab setting
• Administer medications and treatments accurately and document care thoroughly
• Monitor residents’ conditions, report changes, and collaborate with the care team
• Support admission, discharge and care planning activities to promote positive outcomes
• Educate residents and families about care plans, safety and medication management
• Maintain a safe, clean environment and follow infection prevention standards

Qualifications

• Active Licensed Practical Nurse (LPN) credentials — licensed or eligible to be licensed in Massachusetts
• Current CPR/BLS certification (preferred if not already held, can often be obtained)
• Comfort working 12-hour day shifts and delivering patient-focused care
• Strong communication skills and ability to collaborate with interdisciplinary teams

Education

• Completion of an accredited LPN/Vocational Nursing program; high school diploma or equivalent required

Knowledge & Skills

• Proficient with medication administration and resident charting
• Familiarity with long-term care and rehabilitation nursing practices
• Skilled in patient assessment, wound care basics, and vital signs monitoring
• Ability to prioritize tasks, multitask in a busy environment, and maintain accurate documentation
• Compassionate bedside manner and strong interpersonal skills
