Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a long-term/rehab setting
- Administer medications and treatments accurately and document care thoroughly
- Monitor residents’ conditions, report changes, and collaborate with the care team
- Support admission, discharge and care planning activities to promote positive outcomes
- Educate residents and families about care plans, safety and medication management
- Maintain a safe, clean environment and follow infection prevention standards
Qualifications
- Active Licensed Practical Nurse (LPN) credentials — licensed or eligible to be licensed in Massachusetts
- Current CPR/BLS certification (preferred if not already held, can often be obtained)
- Comfort working 12-hour day shifts and delivering patient-focused care
- Strong communication skills and ability to collaborate with interdisciplinary teams
Education
- Completion of an accredited LPN/Vocational Nursing program; high school diploma or equivalent required
Knowledge & Skills
- Proficient with medication administration and resident charting
- Familiarity with long-term care and rehabilitation nursing practices
- Skilled in patient assessment, wound care basics, and vital signs monitoring
- Ability to prioritize tasks, multitask in a busy environment, and maintain accurate documentation
- Compassionate bedside manner and strong interpersonal skills
