Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Conduct comprehensive OASIS assessments and complete accurate home health documentation
- Manage and coordinate patient care plans as the Home Health RN Case Manager
- Provide skilled nursing visits, clinical assessments, and patient education in the home
- Collaborate with interdisciplinary team members and community resources to ensure continuity of care
- Communicate clearly with patients and families to support goals and safety in the home environment
- Maintain compliance with home health regulations and agency policies while promoting quality outcomes
Qualifications
- Registered Nurse (RN) with active licensure (ability to practice in Vermont preferred)
- OASIS experience required
- Home health and case management experience required
- Strong documentation and clinical assessment skills
- Prefer an experienced travel nurse
- Availability for the full contract period with no requested time off
Education
- Graduate of an accredited nursing program (ADN or BSN) and current RN licensure
Knowledge & Skills
- Proficiency with OASIS assessments and home health documentation
- Case management and care coordination skills
- Skilled nursing assessment and clinical decision-making
- Patient and family education in the home setting
- Comfortable using electronic medical records and clear clinical communication
- Time management and ability to manage a caseload independently
