Shift Type: Monday-Friday, 8:00 AM - 4:00 PM (8-hour days; flexible start between 6:00 AM and 10:00 AM), Day shift
Hours per Week: 35
Candidate Pay Rate: $58 per hour

Roles & Responsibilities
- Evaluate and screen patients with communication, cognitive, or swallowing disorders and create individualized plans of care
- Use standardized assessments and evidence-based interventions to guide treatment
- Monitor patient progress and adjust treatment plans, documenting changes in the electronic medical record
- Provide patient, family, and caregiver counseling to support functional outcomes
- Prepare comprehensive discharge summaries and ensure clinical documentation meets regulatory standards
- Accurately record treatment minutes and maintain patient records according to company policy
- Communicate effectively with supervisors and interdisciplinary team members about progress and barriers
- Participate in patient care conferences, utilization review, and rehabilitation meetings
- Obtain necessary DME or supplies as clinically indicated
- Supervise and mentor CFY and student Speech-Language Pathologists
Qualifications
- Active Texas state licensure (required)
- ASHA CCC (required)
- Minimum 1 year of SLP experience (required) — new grads considered if they have Level II experience
- LTC or Rehab experience (required)
- Ability to supervise CFY/Student SLPs
- Three years experience holding active CCS preferred
- Comfortable with electronic medical record documentation and treatment minute recording
Knowledge & Skills
- Assessment and treatment of communication, cognitive, and swallowing disorders
- Proficiency with standardized assessments and evidence-based practice
- Dysphagia management and treatment planning
- Strong clinical documentation skills in an electronic medical record
- Interdisciplinary communication and patient/family counseling
- Experience with utilization review and patient care conferences
- DME procurement processes
- Supervision and mentoring of CFY/Students
- Adherence to ASHA and state practice acts and professional ethics
