Physical Therapist - Home Health
Location: Red Bluff, California

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes throughout Tehama County
- Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
- Develop personalized treatment plans for orthopedic joint replacements and geriatric patients with multiple comorbidities
- Complete approximately 5 meaningful patient visits per day while building lasting relationships
- Document care using HomeCare HomeBase EMR on your provided tablet
- Collaborate with the intake RN and office manager to coordinate exceptional patient outcomes
Qualifications
- Current Physical Therapy license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
- Valid driver's license and reliable transportation for home visits
- Strong communication and interpersonal skills for patient-centered care
Knowledge & Skills
- Physical Therapy expertise in home health settings
- HomeCare HomeBase EMR system proficiency
- Experience with orthopedic and geriatric patient populations
- Ability to manage a 40-mile daily mileage territory efficiently
- Strong clinical assessment and care planning abilities
