Roles & Responsibilities

Provide compassionate physical therapy services directly in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs and create personalized care plans
Guide patients and families through rehabilitation exercises and the safe use of therapeutic equipment
Monitor progress and communicate updates to physicians while celebrating every milestone
Collaborate with home health aides and caregivers to ensure seamless, well-rounded support
Document clinical notes and progress summaries that truly reflect each patient's journey

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits
Two supervisory references from your modality (one within the last 12 months)

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and functional evaluation
Rehabilitation program development
Caregiver education and training
Compliance with Medicare, Medi-Cal, and Title 22 regulations
