Roles & Responsibilities
- Provide compassionate care coordination for patients in need of mental health support
- Conduct thorough assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure seamless patient care
- Support patients and families through challenging healthcare decisions
- Maintain accurate documentation while building meaningful connections with those you serve
Qualifications
- Active LCSW license in California
- Current BLS certification
- 5150 certification in California
- Prior experience with Dignity Health facilities
Knowledge & Skills
- Expertise in involuntary psychiatric evaluation processes
- Strong understanding of mental health crisis intervention
- Excellent communication and documentation abilities
- Ability to work collaboratively within a care coordination model
