Location: Valley City, North Dakota

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes
- Help patients recover from surgeries, falls, and other conditions with personalized care plans
- Collaborate with interdisciplinary teams to ensure the best outcomes for Home Health and Hospice patients
- Travel within a 60-mile radius around Valley City to deliver care where patients need it most
- Document patient progress using HomeCare/HomeBase EMR
Qualifications
- Current ND Physical Therapy License
- BLS certification
- Valid Driver's License and Auto Insurance
- At least 1 year of previous home health experience
- Strong PT-related clinical skills
Knowledge & Skills
- Home health physical therapy expertise
- Comfortable working independently in patient homes
- Familiarity with HomeCare/HomeBase EMR
- Excellent communication and time-management skills
