Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents in a skilled nursing and rehab setting
• Administer medications and document treatments accurately and on time
• Perform routine assessments and report changes in condition to the care team
• Collaborate with nurses, therapists, and physicians to deliver coordinated care plans
• Assist with ADLs and promote resident independence and dignity
• Support a positive resident experience through clear communication and respectful interactions
• Participate in facility documentation, quality initiatives, and safety protocols

Qualifications

• Active LPN license in Massachusetts required
• Completion of an accredited practical nursing program
• Current BLS/CPR certification required (or willingness to obtain)
• State-required immunizations and ability to pass background checks
• Previous experience in long-term care or rehabilitation preferred
• Strong interpersonal and communication skills; team player attitude

Education

• Completion of an accredited Practical Nursing program (LPN diploma or equivalent)
• Active Massachusetts LPN license

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and infection control principles
• Familiarity with electronic medical records and charting
• Assessment skills for older adults and rehab patients
• Time management, prioritization, and delegation in a fast-paced setting
• Empathy, cultural sensitivity, and resident-centered care approach
