Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing/rehab setting
- Administer medications and treatments safely and document thoroughly
- Perform routine assessments and report changes in condition to the care team
- Collaborate with nurses, therapists, and support staff to create and follow individualized care plans
- Assist with activities of daily living while promoting resident independence and dignity
- Maintain accurate, timely charting in the facility’s documentation system
Qualifications
- Active LPN license in the state of Massachusetts (or compact equivalent where applicable)
- Current CPR/BLS certification (or willingness to obtain promptly)
- Previous experience in long-term care, rehab, or skilled nursing is a plus
- Strong communication skills and a team-first attitude
- Reliable, compassionate, and patient-focused work ethic
Education
- Completion of an accredited LPN/LVN program and active state LPN license
Knowledge & Skills
- Clinical skills in medication administration and basic nursing procedures
- Ability to conduct resident assessments and recognize changes in condition
- Familiarity with care planning and interdisciplinary teamwork
- Competence with electronic charting and timely documentation
- Excellent interpersonal skills for resident-family-staff interactions
