Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing/rehab setting
• Administer medications and treatments accurately and document care in a timely manner
• Perform routine assessments and communicate changes in patient condition to the care team
• Collaborate with nurses, therapists and support staff to deliver individualized care plans
• Assist with activities of daily living to promote patient dignity and independence
• Maintain clear, professional documentation and follow facility policies and procedures

Qualifications

• Licensed Practical Nurse (LPN) — current licensure required
• Completion of an accredited practical nursing program
• Comfortable working 12-hour day shifts (8:00 AM - 8:00 PM) in a skilled nursing/rehab environment
• Professional, team-oriented attitude and strong communication skills

Education

• Completion of an accredited LPN/LVN program; high school diploma or equivalent

Knowledge & Skills

• Foundational clinical skills for long-term care and rehabilitation nursing
• Medication administration and accurate charting
• Basic assessment of vital signs and resident status
• Effective communication with residents, families and interdisciplinary teams
• Safety-focused approach and attention to detail
