Roles & Responsibilities
- Provide compassionate, direct nursing care to residents in a skilled nursing setting
- Administer medications and treatments accurately and document care in the patient chart
- Perform thorough assessments and communicate changes in condition to the care team
- Collaborate with RNs, therapists, and physicians to create and follow individualized care plans
- Assist with ADLs, mobility support, and restorative care to promote resident independence
- Maintain a safe, clean environment and follow infection control and facility policies
- Participate in shift handoffs and ensure clear, timely documentation for continuity of care
Qualifications
- Active LPN license in Massachusetts or eligible to obtain licensure
- Completion of an accredited practical/vocational nursing program
- Current BLS/CPR certification (or willing to obtain upon hire)
- Comfortable administering medications and performing routine nursing procedures
- Excellent communication skills and a team-oriented mindset
- Previous experience in long-term care or rehab settings is a plus
Education
- Completion of an accredited LPN / Practical Nursing program
Knowledge & Skills
- Skilled in medication administration and wound care fundamentals
- Strong resident assessment and documentation abilities
- Familiarity with infection prevention and safety best practices
- Ability to collaborate effectively with interdisciplinary teams
- Time management and prioritization in a fast-paced clinical environment
