Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents, focusing on safety and dignity
• Administer medications and treatments accurately and on schedule
• Perform thorough assessments and document changes in patient condition
• Collaborate with nursing staff, therapists, and physicians to execute individualized care plans
• Support rehabilitation goals and assist with ADLs to promote patient independence
• Maintain infection control best practices and ensure a clean, safe care environment
• Communicate clearly with families and care teams to keep everyone informed and empowered

Qualifications

• Current Licensed Practical Nurse credential (LPN)
• Valid state nursing license preferred for Massachusetts; ability to obtain MA licensure if required
• Basic Life Support (BLS) certification or willingness to obtain prior to start
• Previous long-term care, rehab, or skilled nursing experience preferred
• Strong communication skills, clinical judgment, and a team-oriented mindset

Education

• LPN diploma, vocational nursing certificate, or equivalent

Knowledge & Skills

• Medication administration and documentation accuracy
• Wound care and basic rehab support
• Familiarity with electronic health records and charting
• Ability to prioritize tasks in a fast-paced skilled nursing environment
• Compassionate patient-centered care and effective family communication
