Roles & Responsibilities

• Deliver high-quality, compassionate nursing care to residents in a skilled rehab and nursing setting
• Administer medications and treatments safely and accurately
• Monitor and document vital signs, changes in condition, and care provided in the medical record
• Collaborate with RNs, therapists, and the interdisciplinary team to support resident recovery goals
• Assist with rehabilitative activities and daily living tasks to promote independence
• Educate residents and families with clear, respectful communication
• Maintain infection control standards and safety protocols at all times

Qualifications

• Current LPN license in Massachusetts (required)
• Practical nursing diploma or equivalent education
• Current Basic Life Support (BLS) certification preferred or willing to obtain
• Comfortable working in a skilled nursing/rehab environment and collaborating with an interdisciplinary team
• Reliable, compassionate, and detail-oriented with strong documentation habits

Education

• Practical Nursing diploma or certificate (LPN)

Knowledge & Skills

• Medication administration and accurate charting
• Resident assessment and monitoring of clinical changes
• Basic wound care and infection control practices
• Familiarity with electronic medical records and documentation
• Strong communication and teamwork skills with residents, families, and staff
• Ability to prioritize tasks in a fast-paced clinical setting
