Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents at the skilled nursing facility
- Administer medications and treatments accurately and document per facility policy
- Perform routine assessments and report clinical changes to the interdisciplinary team
- Collaborate with nurses, therapists and physicians to implement individualized care plans
- Support resident mobility, activities of daily living and comfort, promoting dignity and independence
- Maintain clear, timely charting in the electronic medical record and assist with care coordination
- Participate in shift handoffs and contribute to a positive, safety-focused work environment
Qualifications
- Current Licensed Practical Nurse (LPN) license (Massachusetts or compact state as applicable)
- Current Basic Life Support (BLS) / CPR certification
- Experience in long-term care, rehabilitation, or skilled nursing preferred
- Ability to pass required background checks, drug screen and health screenings
- Strong communication skills and a team-oriented mindset
Education
- Graduate of an accredited Practical Nursing program or equivalent vocational training
Knowledge & Skills
- Medication administration and IV basics for LPN scope where applicable
- Wound care and basic clinical assessment skills
- Familiarity with long-term care and rehab nursing standards
- Accurate, timely documentation and charting in electronic medical records
- Clinical prioritization, time management and effective patient/family communication
