Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents at the skilled nursing facility
• Administer medications and treatments accurately and document per facility policy
• Perform routine assessments and report clinical changes to the interdisciplinary team
• Collaborate with nurses, therapists and physicians to implement individualized care plans
• Support resident mobility, activities of daily living and comfort, promoting dignity and independence
• Maintain clear, timely charting in the electronic medical record and assist with care coordination
• Participate in shift handoffs and contribute to a positive, safety-focused work environment

Qualifications

• Current Licensed Practical Nurse (LPN) license (Massachusetts or compact state as applicable)
• Current Basic Life Support (BLS) / CPR certification
• Experience in long-term care, rehabilitation, or skilled nursing preferred
• Ability to pass required background checks, drug screen and health screenings
• Strong communication skills and a team-oriented mindset

Education

• Graduate of an accredited Practical Nursing program or equivalent vocational training

Knowledge & Skills

• Medication administration and IV basics for LPN scope where applicable
• Wound care and basic clinical assessment skills
• Familiarity with long-term care and rehab nursing standards
• Accurate, timely documentation and charting in electronic medical records
• Clinical prioritization, time management and effective patient/family communication
