Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a rehab and long-term care setting
- Administer medications and treatments safely and accurately, following facility protocols
- Document resident care thoroughly and timely in medical records
- Monitor resident status and communicate changes promptly to the interdisciplinary team
- Support rehabilitation plans by assisting with ADLs and therapeutic activities
- Educate residents and families about care plans and discharge expectations
- Contribute to a positive, resident-centered environment that prioritizes dignity and recovery
Qualifications
- Active LPN license (Licensed Practical Nurse) — Massachusetts licensure required
- Current BLS/CPR certification preferred or ability to obtain upon hire
- Up-to-date immunizations and ability to pass required health screenings
- Comfortable working in a rehab/long-term care environment; strong teamwork and communication skills
- Previous experience in skilled nursing or rehabilitation settings is a plus
Education
- Graduate of an accredited Practical Nursing program (LPN/LVN diploma or certificate)
Knowledge & Skills
- Medication administration and basic clinical assessments
- Wound care basics and infection control practices
- Accurate charting and documentation in resident medical records
- Ability to follow care plans and collaborate with therapists and nursing staff
- Strong interpersonal skills for resident and family communication
