Roles & Responsibilities

• Provide high-quality bedside nursing care as a Licensed Practical Nurse, prioritizing patient safety and comfort
• Administer medications, treatments, and document thoroughly and accurately
• Perform wound care, IV care as applicable, and monitor vital signs closely
• Collaborate with RNs, providers, and interdisciplinary team members to support individualized care plans
• Communicate clearly with residents and families to educate, advocate, and build trust
• Maintain accurate clinical documentation in the facility’s charting system
• Support admission, transfer, and discharge processes with professionalism and empathy

Qualifications

• Current LPN license in Massachusetts (or compact/eligible where applicable)
• Completion of an accredited LPN program
• Current CPR/BLS certification (or willingness to obtain promptly)
• Valid state-required immunizations and background checks per facility policy
• Comfortable working 12-hour day shifts and collaborating in a rehab/long-term care setting
• Excellent communication and basic EHR/documentation skills

Education

• Graduate of an accredited LPN/LVN program; high school diploma or equivalent required

Knowledge & Skills

• Strong medication administration and documentation skills
• Skilled in basic nursing assessments, wound care, and pressure injury prevention
• Knowledgeable in infection prevention and resident safety practices
• Good time management and prioritization during busy 12-hour shifts
• Team-oriented mindset with the ability to follow care plans and escalate concerns
