Roles & Responsibilities
- Provide high-quality bedside nursing care as a Licensed Practical Nurse, prioritizing patient safety and comfort
- Administer medications, treatments, and document thoroughly and accurately
- Perform wound care, IV care as applicable, and monitor vital signs closely
- Collaborate with RNs, providers, and interdisciplinary team members to support individualized care plans
- Communicate clearly with residents and families to educate, advocate, and build trust
- Maintain accurate clinical documentation in the facility’s charting system
- Support admission, transfer, and discharge processes with professionalism and empathy
Qualifications
- Current LPN license in Massachusetts (or compact/eligible where applicable)
- Completion of an accredited LPN program
- Current CPR/BLS certification (or willingness to obtain promptly)
- Valid state-required immunizations and background checks per facility policy
- Comfortable working 12-hour day shifts and collaborating in a rehab/long-term care setting
- Excellent communication and basic EHR/documentation skills
Education
- Graduate of an accredited LPN/LVN program; high school diploma or equivalent required
Knowledge & Skills
- Strong medication administration and documentation skills
- Skilled in basic nursing assessments, wound care, and pressure injury prevention
- Knowledgeable in infection prevention and resident safety practices
- Good time management and prioritization during busy 12-hour shifts
- Team-oriented mindset with the ability to follow care plans and escalate concerns
