Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
- Administer medications and treatments accurately and document according to facility policy
- Perform regular assessments and communicate changes in patient status to the care team
- Assist with activities of daily living while promoting resident dignity and independence
- Collaborate with RN leadership, therapists and support staff to coordinate individualized care plans
- Maintain clear, timely charting and follow infection control and safety protocols
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing
- Completion of an accredited practical nursing program
- Current Basic Life Support (BLS/CPR) certification preferred
- Comfortable working in a long-term care or rehabilitation environment; experience is a plus
- Strong communication skills and the ability to work collaboratively with an interdisciplinary team
Education
- Graduate of an accredited practical nursing (LPN) program
Knowledge & Skills
- Clinical skills in medication administration, wound care and basic nursing procedures
- Ability to perform resident assessments and recognize changes in condition
- Competence with electronic or paper charting and documentation standards
- Good time management, prioritization and the ability to multitask in a busy setting
- Person-centered approach with strong interpersonal and problem-solving skills
