Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments accurately and document according to facility policy
• Perform regular assessments and communicate changes in patient status to the care team
• Assist with activities of daily living while promoting resident dignity and independence
• Collaborate with RN leadership, therapists and support staff to coordinate individualized care plans
• Maintain clear, timely charting and follow infection control and safety protocols

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing
• Completion of an accredited practical nursing program
• Current Basic Life Support (BLS/CPR) certification preferred
• Comfortable working in a long-term care or rehabilitation environment; experience is a plus
• Strong communication skills and the ability to work collaboratively with an interdisciplinary team

Education

• Graduate of an accredited practical nursing (LPN) program

Knowledge & Skills

• Clinical skills in medication administration, wound care and basic nursing procedures
• Ability to perform resident assessments and recognize changes in condition
• Competence with electronic or paper charting and documentation standards
• Good time management, prioritization and the ability to multitask in a busy setting
• Person-centered approach with strong interpersonal and problem-solving skills
