Shift Type: Varied days as scheduled, 8:00 AM - 8:00 PM Day shift (12-hour day)

Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Provide compassionate direct patient care including ADLs, vital signs, and treatment administration
- Administer medications and treatments safely and document per facility policies
- Collaborate with the interdisciplinary team to develop and update individualized care plans
- Monitor patient status, recognize changes, and escalate concerns promptly
- Educate patients and families about care plans, medications, and discharge instructions
- Maintain accurate, timely documentation in the electronic health record
- Support a positive, respectful environment that promotes patient dignity and recovery
Qualifications
- Active LPN license in Massachusetts required
- Completion of an accredited LPN / practical nursing program
- Current BLS/CPR certification (or ability to obtain upon hire)
- Previous experience in long-term care, rehab, or skilled nursing preferred
- Up-to-date immunizations and ability to pass background and health screenings
- Strong communication skills and the ability to work well in a team
Education
- Graduate of an accredited Licensed Practical Nurse (LPN) program or equivalent
Knowledge & Skills
- Medication administration and documentation accuracy
- Wound care basics and pressure injury prevention
- Comfortable with electronic health record systems and charting
- Skilled at prioritizing tasks in a fast-paced rehab and nursing environment
- Patient-centered approach with strong clinical judgment and assessment skills
- Excellent interpersonal skills with residents, families, and care teams
