Roles & Responsibilities

• Provide compassionate direct patient care including ADLs, vital signs, and treatment administration
• Administer medications and treatments safely and document per facility policies
• Collaborate with the interdisciplinary team to develop and update individualized care plans
• Monitor patient status, recognize changes, and escalate concerns promptly
• Educate patients and families about care plans, medications, and discharge instructions
• Maintain accurate, timely documentation in the electronic health record
• Support a positive, respectful environment that promotes patient dignity and recovery

Qualifications

• Active LPN license in Massachusetts required
• Completion of an accredited LPN / practical nursing program
• Current BLS/CPR certification (or ability to obtain upon hire)
• Previous experience in long-term care, rehab, or skilled nursing preferred
• Up-to-date immunizations and ability to pass background and health screenings
• Strong communication skills and the ability to work well in a team

Education

• Graduate of an accredited Licensed Practical Nurse (LPN) program or equivalent

Knowledge & Skills

• Medication administration and documentation accuracy
• Wound care basics and pressure injury prevention
• Comfortable with electronic health record systems and charting
• Skilled at prioritizing tasks in a fast-paced rehab and nursing environment
• Patient-centered approach with strong clinical judgment and assessment skills
• Excellent interpersonal skills with residents, families, and care teams
