Roles & Responsibilities

• Evaluate patients' physical function and develop individualized, evidence-based treatment plans
• Deliver compassionate outpatient physical therapy interventions for orthopedic, sports, pediatric and neurological conditions
• Collaborate with interdisciplinary teams and caregivers to support patient goals and safe discharge plans
• Document progress clearly and timely in the medical record, tracking outcomes and modifying plans as needed
• Educate patients and families on home programs, injury prevention and strategies to maximize independence
• Use clinical reasoning to manage diverse caseloads across multiple points of care

Qualifications

• Current unencumbered Oregon Physical Therapy license (required)
• Healthcare Provider Level BLS (required)
• Ability to travel to multiple points of care during the workday (required)
• Experience or interest in outpatient physical therapy and rehabilitation (orthopedic, sports, pediatric, neurological preferred)
• Strong communication skills and ability to work collaboratively with patients and clinical teams

Knowledge & Skills

• Skilled in patient evaluation, functional assessment and treatment planning
• Experience with outpatient services including orthopedic and sports rehab
• Knowledge of neurological rehab, balance and dizziness interventions
• Familiarity with wheelchair seating assessments and adaptive strategies
• Comfortable documenting in electronic medical records and tracking outcomes
