Roles & Responsibilities

• Coordinate patient care across the clinic and with external providers to ensure smooth transitions and continuity
• Manage case assignments, develop individualized care plans, and follow up to track outcomes
• Collaborate with nursing staff and providers to support efficient clinic operations and optimal patient care
• Provide patient education and resources to empower self-management and improve health literacy
• Document care activities accurately and timely in the electronic medical record
• Participate in interdisciplinary team meetings to resolve barriers to care and improve processes
• Assist with care coordination tasks that support population health initiatives and clinic goals

Qualifications

• Active RN license (must be eligible to practice in Arizona; compact license may be acceptable)
• Experience in care coordination, case management, clinic nursing, or related patient-care roles
• Strong clinical assessment and patient advocacy skills
• Excellent communication and collaboration skills when working with providers, patients, and community resources
• Basic life support (BLS) certification preferred or willingness to obtain
• Comfort working within a clinic environment and engaging with diverse patient populations

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Knowledge of case management principles and care coordination workflows
• Skillful patient assessment and care planning
• Proficient documentation practices and comfort with electronic medical records
• Ability to collaborate effectively with interdisciplinary teams
• Cultural sensitivity and experience serving Native American communities a plus
• Organizational skills to manage multiple cases and follow-up tasks
