Roles & Responsibilities

• Assess patients' clinical status regularly and deliver skilled nursing care following the physician-approved plan of care
• Initiate, revise, and adapt individualized plans of care to meet changing patient needs
• Provide comprehensive medication management and monitor therapeutic responses closely
• Coordinate patient care and evaluate outcomes, offering ongoing feedback to the care team to improve patient plans
• Collaborate with CNAs and the interdisciplinary team to ensure safe, high-quality care
• Notify physicians promptly of condition changes and obtain new orders as required
• Document care accurately and thoroughly in the Epic EMR to support continuity and quality of care

Qualifications

• Active Michigan RN license (required)
• BLS (AHA) certification (required)
• Minimum 2+ years of Long Term Care (LTC) nursing experience (required)
• Experience using Epic EMR (important for success in this role)
• Strong teamwork and communication skills for interdisciplinary collaboration
• Profile requirements: current work history, skills checklist (within 1 year), 2 supervisory references (within last 2 years), and permanent address (facility enforces a 100-mile local candidate restriction)

• Patient assessment and clinical decision-making
• Medication management and therapeutic monitoring
• Care planning and individualized interventions
• Care coordination across interdisciplinary teams
• Documentation best practices in Epic EMR
• Ability to collaborate effectively with CNAs and other support staff
• Familiarity with nursing standards, physician orders, and agency policies
