Roles & Responsibilities
- Observe residents continuously and promptly report changes in physical or mental status
- Administer medications and treatments as ordered and document accurately in the MAR/TAR/EMR
- Supervise, assign and mentor LNAs, ensuring care is delivered safely and within scope
- Perform and document resident assessments, admissions, discharges and transfers
- Provide wound care (including familiarity with wound vacs) and manage enteral (G/J-tube) feedings
- Administer oxygen therapy according to facility guidelines and monitor residents' responses
- Support physician rounds and communicate care needs during 60-day reviews and routine care
- Oversee infection control practices and maintain a safe, hazard-free unit environment
- Complete 24-hour/unit reports, review new orders, and maintain communication with nursing leadership
- Engage with residents' families to resolve concerns, relay new orders, and obtain consent when needed
Qualifications
- Active New Hampshire nursing license in-hand at time of submission
- Minimum 3 years of nursing experience; must have worked within the last 6 months
- BLS (AHA) current and active at time of submission
- IV certification required; willing to obtain during onboarding if not already certified
- COVID-19 vaccination required (declinations/exemptions not accepted) and proof must be uploaded
- Willingness to float to units where experienced is required
- Two supervisory references from the last two years of employment
- All certifications and license must be active at time of submission
- Must have reliable transportation due to rural location and limited public transit
- Locals residing within 50 miles of the facility are not eligible
Knowledge & Skills
- Medication administration and computerized MAR/TAR/EMR documentation
- Resident assessment and clinical judgment in long-term care settings
- IV therapy skills and willingness to be IV-certified
- Wound care expertise and familiarity with wound vac systems
- Enteral feeding management for gastrostomy/jejunostomy tubes
- Oxygen administration and monitoring
- Infection control practices and maintaining a safe unit
- Supervisory skills: assigning tasks, coaching LNAs, and staff evaluation
- Strong communication skills for interacting with residents, families and interdisciplinary teams
- Ability to manage unit responsibilities during shift and escalate appropriately
