Shift Type: Monday-Friday, 8:00 AM - 4:00 PM (Day shift); on-call 1-2 nights per week; occasional weekend coverage as needed
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Assess patient needs in the home and develop individualized care plans
- Administer medications safely and monitor patient responses
- Provide compassionate hospice and end-of-life care focused on comfort and quality of life
- Educate patients and families on care plans, medications, and symptom management
- Document clinical care clearly and accurately in the patient record
- Coordinate with interdisciplinary team members to ensure continuity of care
- Monitor patient conditions and escalate concerns promptly to the care team
- Maintain safe driving practices while providing in-home visits
Qualifications
- Current Montana RN license required
- BLS certification required
- Valid driver’s license and clean driving record required
- Experience working in both Home Health and Hospice settings
- Strong patient assessment, documentation, and care-planning experience
Education
- Graduate of an accredited nursing program (ADN or BSN preferred) and active RN license
Knowledge & Skills
- Patient assessment and monitoring in the home setting
- Developing and implementing individualized care plans
- Medication administration and safe practices
- Clinical documentation and electronic charting
- Patient and family education around disease, symptom management, and end-of-life care
- Care coordination across interdisciplinary teams
- Comfort with palliative and end-of-life care principles
