Shift Type: Monday-Friday, 8:00 AM - 4:00 PM (Day shift); on-call 1-2 nights per week; occasional weekend coverage as needed

Roles & Responsibilities

• Assess patient needs in the home and develop individualized care plans
• Administer medications safely and monitor patient responses
• Provide compassionate hospice and end-of-life care focused on comfort and quality of life
• Educate patients and families on care plans, medications, and symptom management
• Document clinical care clearly and accurately in the patient record
• Coordinate with interdisciplinary team members to ensure continuity of care
• Monitor patient conditions and escalate concerns promptly to the care team
• Maintain safe driving practices while providing in-home visits

Qualifications

• Current Montana RN license required
• BLS certification required
• Valid driver’s license and clean driving record required
• Experience working in both Home Health and Hospice settings
• Strong patient assessment, documentation, and care-planning experience

Education

• Graduate of an accredited nursing program (ADN or BSN preferred) and active RN license

Knowledge & Skills

• Patient assessment and monitoring in the home setting
• Developing and implementing individualized care plans
• Medication administration and safe practices
• Clinical documentation and electronic charting
• Patient and family education around disease, symptom management, and end-of-life care
• Care coordination across interdisciplinary teams
• Comfort with palliative and end-of-life care principles
