Roles & Responsibilities

• Assess patients to identify mental, physical and spiritual needs and tailor care accordingly
• Develop, implement and evaluate individualized plans of care in collaboration with the multidisciplinary team
• Provide clear explanations and instructions to patients and responsible parties to promote understanding and recovery
• Use strong clinical judgment to recognize and report changes in patient condition promptly
• Maintain complete, accurate medical records to ensure continuity and quality of care
• Contribute to a healthy workplace by managing conflict professionally and communicating effectively
• Participate in continuing education to stay current with best practices and evidence-based care
• Meet Core Measure responsibilities and help the team maintain compliance and high-quality outcomes

Qualifications

• Current Illinois Registered Nurse (RN) license (required)
• Current CPR/BLS certification (required)
• Experience or comfort working in a Progressive Care Unit (PCU) environment preferred
• Strong patient assessment and care planning skills
• Proven ability to document medical records accurately and thoroughly
• Comfort collaborating with a multidisciplinary healthcare team and providing patient education
• Professional communication and conflict management skills

Education

• Graduate of an accredited nursing program (ADN, BSN, or diploma) or equivalent
