Roles & Responsibilities

• Provide skilled speech-language pathology evaluations and individualized treatment plans for SNF residents
• Deliver evidence-based therapy that improves communication, cognition, and swallowing function
• Document patient progress accurately and timely to meet facility and CRG policies
• Collaborate closely with the therapy team, nursing staff, and interdisciplinary partners to optimize outcomes
• Maintain therapist productivity expectations (90%) while prioritizing patient-centered care
• Be present in the building during scheduled hours to receive guaranteed patient visits

Qualifications

• Licensed SLP (state license requirements apply; please have documentation ready)
• 1-3 years of experience working in skilled nursing or therapy settings (per job posting)
• Comfort with SNF workflows, documentation systems, and interdisciplinary collaboration
• Willingness to follow Continuum Rehabilitation Group policies and facility dress code (black scrubs)
• Reliable on-site presence and strong time-management to meet productivity expectations

Knowledge & Skills

• Skilled Nursing Facility experience and familiarity with long-term care dynamics
• Treatment planning for speech, language, cognition, and dysphagia in post-acute populations
• Strong clinical documentation and outcome tracking
• Teamwork and clear communication with nursing and rehab staff
• Ability to follow facility policies and maintain professional appearance (black scrubs)
