Roles & Responsibilities
- Provide skilled speech-language pathology evaluations and individualized treatment plans for SNF residents
- Deliver evidence-based therapy that improves communication, cognition, and swallowing function
- Document patient progress accurately and timely to meet facility and CRG policies
- Collaborate closely with the therapy team, nursing staff, and interdisciplinary partners to optimize outcomes
- Maintain therapist productivity expectations (90%) while prioritizing patient-centered care
- Be present in the building during scheduled hours to receive guaranteed patient visits
Qualifications
- Licensed SLP (state license requirements apply; please have documentation ready)
- 1-3 years of experience working in skilled nursing or therapy settings (per job posting)
- Comfort with SNF workflows, documentation systems, and interdisciplinary collaboration
- Willingness to follow Continuum Rehabilitation Group policies and facility dress code (black scrubs)
- Reliable on-site presence and strong time-management to meet productivity expectations
Knowledge & Skills
- Skilled Nursing Facility experience and familiarity with long-term care dynamics
- Treatment planning for speech, language, cognition, and dysphagia in post-acute populations
- Strong clinical documentation and outcome tracking
- Teamwork and clear communication with nursing and rehab staff
- Ability to follow facility policies and maintain professional appearance (black scrubs)
