Roles & Responsibilities
- Provide skilled occupational therapy evaluations and develop individualized treatment plans for SNF residents
- Deliver hands-on therapeutic interventions and adaptive strategies to improve resident independence
- Document clinical progress thoroughly and timely in accordance with facility and CRG policies
- Collaborate closely with nursing, PT/SLP, and interdisciplinary team members to coordinate care
- Meet productivity expectations while maintaining high-quality, patient-centered care (therapist productivity requirement: 90%)
- Participate in facility-specific processes and follow CRG policies & procedures
- Maintain professional presence on-site (dress code: black scrubs) and support a positive therapy culture
Qualifications
- 1-3 years of Occupational Therapy experience (as noted in the job posting)
- Active Occupational Therapist license or eligibility to obtain licensure in New Mexico
- Experience in a skilled nursing facility (SNF) or similar post-acute setting preferred
- Strong documentation skills and ability to meet productivity expectations
- Excellent communication and teamwork skills; able to collaborate with an interdisciplinary team
- Willingness to follow CRG policies & procedures and meet facility dress code
Knowledge & Skills
- Skilled in OT assessments, treatment planning, and skilled interventions for SNF residents
- Knowledge of SNF regulations, documentation standards, and productivity expectations
- Proficiency with clinical documentation systems and timely charting
- Ability to provide patient-centered, functional interventions that focus on independence
- Strong interpersonal skills for team collaboration and family communication
- Adaptability to the facility environment and adherence to site-specific policies
