Roles & Responsibilities

• Provide high-quality physical therapy services to skilled nursing facility residents, focusing on safe, functional outcomes
• Assess, evaluate, and document patient status and progress in accordance with facility policies
• Develop and implement individualized treatment plans that address mobility, strength, balance, and fall prevention
• Collaborate with the therapy team and interdisciplinary staff to coordinate care and optimize patient outcomes
• Maintain required productivity expectations and follow CRG policies and procedures
• Participate in team meetings and contribute to a positive, resident-centered therapy culture
• Adhere to the facility dress code and onsite presence requirements to support guaranteed hours

Qualifications

• PT or DPT credential (PT/DPT experience noted in the job requirements)
• 1-3 years of professional experience (experience in SNF settings is relevant)
• Comfort working onsite and meeting facility expectations for presence and productivity
• Ability to follow company policies and procedures (CRG P&P) and meet productivity targets
• Professional communication skills and a collaborative approach with teammates and leadership

Education

• PT or DPT credential (as indicated by job requirements)

Knowledge & Skills

• Clinical expertise in physical therapy for skilled nursing/residential populations
• Skilled in assessment, treatment planning, and documentation for SNF patients
• Strong time management and productivity awareness to meet facility targets
• Collaborative teamwork and effective communication with interdisciplinary staff
• Adaptability to facility routines and resident-centered care models
• Comfortable with facility protocols and a focus on safety and quality
