Roles & Responsibilities
- Provide high-quality physical therapy services to skilled nursing facility residents, focusing on safe, functional outcomes
- Assess, evaluate, and document patient status and progress in accordance with facility policies
- Develop and implement individualized treatment plans that address mobility, strength, balance, and fall prevention
- Collaborate with the therapy team and interdisciplinary staff to coordinate care and optimize patient outcomes
- Maintain required productivity expectations and follow CRG policies and procedures
- Participate in team meetings and contribute to a positive, resident-centered therapy culture
- Adhere to the facility dress code and onsite presence requirements to support guaranteed hours
Qualifications
- PT or DPT credential (PT/DPT experience noted in the job requirements)
- 1-3 years of professional experience (experience in SNF settings is relevant)
- Comfort working onsite and meeting facility expectations for presence and productivity
- Ability to follow company policies and procedures (CRG P&P) and meet productivity targets
- Professional communication skills and a collaborative approach with teammates and leadership
Education
- PT or DPT credential (as indicated by job requirements)
Knowledge & Skills
- Clinical expertise in physical therapy for skilled nursing/residential populations
- Skilled in assessment, treatment planning, and documentation for SNF patients
- Strong time management and productivity awareness to meet facility targets
- Collaborative teamwork and effective communication with interdisciplinary staff
- Adaptability to facility routines and resident-centered care models
- Comfortable with facility protocols and a focus on safety and quality
