Roles & Responsibilities

• Conduct skilled speech-language and swallowing evaluations and develop individualized treatment plans
• Provide compassionate, evidence-based therapy to skilled nursing residents
• Document progress clearly and timely to meet facility and regulatory standards
• Collaborate with the interdisciplinary therapy and nursing team to optimize patient outcomes
• Meet productivity expectations and contribute positively to team performance
• Adhere to Continuum Rehabilitation Group policies and facility protocols
• Maintain professional appearance and follow dress code (black scrubs)

Qualifications

• Active SLP license in New Mexico or eligibility to obtain state licensure
• 1-3 years of clinical experience (SNF experience preferred)
• Comfortable working day shift schedules in a skilled nursing facility
• Strong communication skills and ability to work collaboratively with a therapy team
• Willingness to follow facility policies and meet productivity expectations

Education

• Master's degree in Speech-Language Pathology (or equivalent professional credential)

Knowledge & Skills

• Skilled assessment and treatment of speech, language, cognition, and swallowing disorders
• Experience with documentation and clinical productivity expectations
• Ability to create meaningful, person-centered treatment plans
• Good time management, organization, and clinical decision-making
• Familiarity with SNF workflows and interdisciplinary collaboration
