Roles & Responsibilities
- Administer medications and treatments safely and document accurately
- Monitor and record patient vital signs and clinical changes
- Provide compassionate personal care and support activities of daily living
- Collaborate with the interdisciplinary team to implement individualized care plans
- Assist with admissions, transfers, and discharges to ensure smooth transitions
- Maintain thorough, timely charting in the facility’s record system
- Support rehabilitative and restorative nursing programs to improve patient outcomes
- Adhere to infection control standards and facility policies to maintain a safe environment
Qualifications
- Current Massachusetts LPN license required
- BLS/CPR certification required or obtained upon hire
- Previous experience in long-term care or rehab settings preferred
- Strong communication and teamwork skills; dependable and patient-focused
- Ability to pass background checks and required health screenings
Education
- Graduate of an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Medication administration and accurate documentation
- Basic wound care and safe patient handling
- Familiarity with electronic health records and point-of-care charting
- Knowledge of infection control practices and safety protocols
- Ability to prioritize tasks and respond to changing clinical needs
